What the template records

Use one row per service event. Keep an asset ID consistent across rows so you can filter an individual item's history.

  • Identification: asset ID, equipment name, brand and model.
  • When and how much it was used: service date, usage reading and unit, such as hours or miles when applicable.
  • What happened: service or repair, work performed, part number and who completed the work.
  • Cost and evidence: parts cost, labor cost and a reference to the receipt, work order or photo.
  • Next action: next due date, next usage threshold and follow-up notes.

Fill the first row in five steps

  1. Give the item an asset ID, such as TOOL-001. Record its actual model separately from its serial number.
  2. Enter the service date and the usage reading available on that date. Leave unknown values blank rather than guessing.
  3. Describe the work in plain language. Separate a completed repair from a proposed diagnosis or an item awaiting approval.
  4. Record actual costs and a reference to the supporting document. The blank template has no currency conversion or tax calculations.
  5. Set the next date or usage threshold using the manufacturer's instructions and your actual operating conditions. A blank field means the next interval has not been confirmed.

A fictional example

The example file uses a fictional workshop vacuum and a recorded inspection with a follow-up date. The dates and tasks illustrate how to fill out the log. They are not a maintenance schedule for any real machine or model.

Calendar dates and operating hours are different

Equipment may have calendar-based, usage-based or combined service requirements. Keep date and usage thresholds in separate columns. Check the manufacturer's current instructions before deciding when service is due. This template does not calculate or prescribe service intervals.

Keep identifying information private

The file downloads directly to your device. Entered equipment details stay in the spreadsheet you choose; this page does not receive them. A spreadsheet is not automatically encrypted. Limit access, protect backup copies and avoid putting full serial numbers or customer contact information into a publicly shared sheet.

For a separate identification record, use the serial-number and ownership checklist. For a cleaner record of recurring issues, read the repair and maintenance history guide.

When you want records connected to the equipment

StuffDNA brings equipment profiles, maintenance history and repair work into one workshop workflow. Explore StuffDNA and its current availability, or use Tool Protection to learn about ownership records and good-faith registry checks.

For hazardous work, follow manufacturer instructions and use a qualified technician when needed. A completed log is a record of what someone entered, not proof that equipment is safe to operate.
